Incident Report Book
The Incident Report Book is the Incident Report Book tab on the Centralized Reports page at /centralized-reports?tab=incident-report-book. It assembles a single, board-ready dossier of incidents and concerns linked to your AI systems and use cases.
What the report includes
- Per-system summary — incident count, last-incident date, severity mix
- Per-incident detail — status, severity, summary, and linkage to the source AIREP intake
- Concerns rollup — anonymous-channel concerns triaged into incidents (A.3.3)
- Containment & closure narrative — sourced from the AI System detail incident playbook
